Mobenzi Outreach Video Walkthrough

Outreach is the newest platform to join the Mobenzi family. It provides a common base on which to configure community-based care requirements.

To help demonstrate the platform’s capabilities, we’ve put together the following short videos highlighting the key processes supported.

The Scenario

A Community Health Worker (CHW) is assigned to a given area and enrols patients into a central database as she performs routine door-to-door visits. Once enrolled, she is then responsible for visiting them on a regular basis. She needs access to historic patient information and to be guided in what activities to complete at each visit and assistance with diagnoses from facility-based specialists.

Video #1: Mobenzi Outreach allows patient records to be defined as required. These records are created during enrollment and updated and added to over time. The patient record could be configured to be comprehensive or partial – integrating into or from third party systems as required.

Video #2: Outreach maintains a list of patients for each CHW. A CHW can schedule their own visits or a visit might be scheduled by a supervisor or automatically according to a schedule.

Video #3: Patient visit forms are completely configurable and are loaded with relevant information regarding the patient to guide the CHW through the visit process.

Video #4: Outreach can be configured with algorithms to assist CHWs in diagnosing and treating certain conditions. If they need assistance from a specialist however, the are able to refer the case from the mobile application.

Video #5: Outreach generates a case file and distributes this to a participating specialist via email (in this implementation). The PDF includes contextual information from the patient’s record as well imagery and observations captured by the CHW in the field. The specialist’s feedback could easily be returned to the CHW via SMS or another mechanism.
